If you do not have a Letter of Appointment or have questions regarding the program, please contact the Family Readiness Assistant at 801-432-4916 or DSN 766-4916. Feel free to use the sample letter by clicking on the link above.
  • Utah National Guard Family Readiness Info Sheet and Volunteer Agreement: Please fill out this form and send a copy to the Family Programs Office. It can be mailed or faxed in to our office. This form allows our office to know who is in the FRG position for your unit. All of the information is required and will be kept in a secure location. We need to have all of your banking information; this allows us to put you on orders and to be reimbursed.
